Application Guide

Notice

Your child may not enter Kindergarten (K-5) until all of their Kindergarten shots have been completed, and proper documentation is turned into the CCA office. Please note that if your child has not begun their Hepatitis Series, this will take several months to complete, so please start on them immediately. All vaccines should be recorded on a DH 680 Form, available through your immunization clinic/doctor’s office, and coded DOE Code 1.

Your child may not enter 7th grade until all their 7th grade shots have been completed, and proper documentation turned in to the CCA office. Please note that if your child has not begun their Hep Series, this will take several months to complete, so you will need to start on them now. 7TH grade shots consist of three Hep shots and one TD booster (Tdap). All shots need to be recorded on a DH 680 form, available through an immunization clinic, and coded DOE Code 8.

Admissions Status Tracking

After your application is submitted, you will be able to track your admissions status at the school, monitor when the school receives any applicable supplemental forms and, if necessary, reprint the forms and completed application. To view your application status, please use your username and password to access your account.

Pastoral Reference Form

 This form was emailed to the reference you listed in your application.  Please ask them to check both their Inbox and Spam emails for the form to complete it.  Once they submit it, it will automatically be emailed back to the school.  

School Reference Form

This form was faxed or emailed to your student's most recent school.  Please reach out to them to encourage them to complete and return the form to us asap.  

Report Card/Transcript

You can email or drop off your student's most recent report card and a copy of their most recent test scores.  (This was also requested from their most recent school; but there may be a delay in receiving it from them)

IEP or 504: (if applicable)

You can email or drop off your student's IEP or 504 to us asap for review.
